Hi and welcome,
This page is for new & existing members of Shepshed Swimming Club and has attachments for forms that members either have to fill in and return to the club or are for information regarding club rules,fire drills etc. The two forms which can be printed off are the Application Form & Medical Form, these need to be completed and returned back to the club.
If you are a new member you need to fill in one extra form but unfortunately I cannot put it on the net, this is for the A.S.A and is a necessary insurance form which has to be sent back to them, this form will be given to you separately.
